Confirmation of accommodation for the visa

Terms and conditions of accommodation:

  1. Send the valid accommodation application by 15.7. (winter semester – (Ws)) at the latest, or by 15.12 (summer semester (Ss)) to the e-mail address: accommodation@vsb.cz or by fax +420 59 699 6499.
  2. You will be informed by e-mail about the decision regarding your acceptance by 20.8.(Ws) at the latest or by 20.1.(Ss). You will receive information regarding payment of a deposit to an amount of CZK 5000. The deposit must be paid by 31.8. (Ws) at the latest, or by 31.1.(Ss). The amount must on at our account at this time at the latest. Your claim for accommodation expires in case of non-payment of the deposit by this date. The deposit will be returned upon termination of accommodation and after payment of all outstanding debts.
  3. If you do not arrive for accommodation by the date of your accommodation contract or fail to inform us regarding changes by 31.8.(Ws) at the latest or by 31.1.(Ss), the deposit will be used as a replacement for the booked accommodation place and for administrative costs.

I, the undersigned, confirm the truthfulness and completeness of the information above and am aware of the fact that all missing and untrue statements will result in the invalidation of my accommodation form. I do confirm, that I was familiarized with the terms and conditions of accommodation. With my signature, I indicate my agreement for manipulation and use of my personal statements for the purposes of managing the accommodation form and maintaining records regarding the accommodated subjects. The personal statements are kept in accordance with law no. 101/2000 , for 5 years at the longest.
