Summary
The course explains the process of investigation of accidents and incidents, elaborates on the typology of accidents and obtaining information about the mechanism of the accident in wider context, including the proper operation of the process and root causes. It provides information on the steps, evidence and procedures needed in the investigation, from the formation of the team throw the creation and verification of hypotheses to the processing of lessons learned from the accident. The subject is supplemented by case studies.
Learning Outcomes of the Course Unit
Understand and apply basic methods of determining the course of incidents and accidents and their causes (including root causes) and context. Apply incident and accident investigation methods to specific situations and draw lessons learned from them.
Course Contents
1. What is an accident and accident investigation and what is it for?
2. Typology of accidents
3. Process of investigation of accidents
4. Obtaining information about the course of the accident and its context, including the proper operation of the process
5. Analysis of information obtained
6. Evidence and hypothesis, hypothesis evaluation
7. Confirmation and refutation of hypotheses
8. Identification of risk control mechanisms and their failure
9. Methods of analysis: DOE, FTA, ETA,
10. Methods of analysis: MORT, security barriers analysis
11. Primary and root causes, their relationship and importance
12. Lessons learned
13. Action plan and its implementation. Importance of accident and accident investigation for enterprise, industrial segment and for society.
