Investigation of Incidents and Accidents

Summary

The course explains the process of investigation of accidents and incidents, elaborates on the typology of accidents and obtaining information about the mechanism of the accident in wider context, including the proper operation of the process and root causes. It provides information on the steps, evidence and procedures needed in the investigation, from the formation of the team throw the creation and verification of hypotheses to the processing of lessons learned from the accident. The subject is supplemented by case studies.
