Based on an extraordinary measure of the Ministry of Health of the Czech Republic, this order of the Rector, with effect from 10 May 2021 00:00, restricts the operation of universities so that:

If you are a student of VSB-TUO and you have a positive test for COVID-19 or you have been ordered quarantine, fill in this information in information system:
Universities have been required to provide students with antigenic tests to determine the presence of SARS-CoV-2 virus or RT-PCR tests for the presence of SARS-CoV-2 virus in the body.
This obligation does not apply to students who participate in an individual consultation (one student, one academic staff member) and are not accommodated in dormitories. At the entrance to the building, such students will prove themselves by a completed Statutory Declaration, which is available at the gatehouse or for download here, Statutory declaration in connection with entering the University, and which they hand over to the person with whom the consultation is arranged.
Personal presence at the permitted education (teaching, examination) is thus allowed to the student only if the following conditions are met. If he/she:

Upon entering the building, the student must prove himself/herself with a student ID card, whose identity will be verified via a card reader by an authorised person.
In case the student uses the possibility of self-testing provided by VSB-TUO, it is necessary to proceed as follows:
A – student accommodated in dormitory
B – other students
The student is obliged to inform the VSB-TUO that his/her test result is positive, and the VSB-TUO is obliged to issue a confirmation of a positive result to the student.
If the test result is positive, the student is obliged to immediately leave the premises of the VSB-TUO or another place where education takes place and the premises of the accommodation facility of the VSB-TUO. If it is his/her place of residence, he/she does not have to leave the accommodation but will enter the quarantine.
The student is obliged to inform the provider of health services in the field of general practice by telephone or other usual remote means about the positive test result and follow their instructions.
Registration for academic staff of universities in the central reservation system crs.mzcr.cz will be possible from 3 May 2021 to 15 June 2021. The code assigned to VSB-TUO by the Ministry of Health serves as the code for registration in this system. With this code, the academic staff member will register regardless of the faculty at which he/she works. It is confidential information for which it is necessary to prevent unauthorised use, publishing or provision outside the circle of persons to whom it is intended.
The launch of the vaccination registration applies to all academic staff (i.e. staff involved in teaching students, especially in practical, laboratory, experimental teaching, etc.) of public and private universities.
After a specific academic staff member registers for vaccination in the central reservation system, he/she must have a confirmation of employment issued, using a uniform format. The relevant personnel officer will issue the certificate of employment upon request. With this confirmation, the academic staff member will subsequently prove himself/herself at the vaccination centre, where he/she will be verified against an identity card or other identity document. Without this confirmation, vaccination will not be possible.
Through the information line 1221 and the e-mail email@example.com, support will be provided to academic staff in case of problems with logging in to the central reservation system. The 1221 line is also used for public inquiries related to the COVID-19 pandemic. It is open every day, including weekends, from 8 am to 7 pm.
In case the employee has a positive result of the test for the presence of SARS-CoV-2 virus antigen, he/she is obliged to inform his/her immediate superior as soon as possible, if he/she is present at the workplace, he/she has to leave it immediately, return to the place of his/her current residence and immediately notify the result of the test to his/her doctor or public health authority (Regional Hygiene Station of the Moravian-Silesian Region with its registered office in Ostrava).
Testing applies to all employees (i.e., also those who have concluded an agreement with VSB-TUO on work performance or a contract on work activities).
Testing will take place:
There will primarily be used tests that are gentle on the nasal mucosa by antigen testing and are completely painless (nasal or saliva tests, if possible).
Employees, except employees of the faculties where self-testing will occur exclusively, are obliged to register for testing through the reservation system. The reservation system will be available at https://reservatic.com/cs. It will not be possible to undergo testing without registration.
The reservation system will be launched on 12 March 2021. After its launch, registered employees will receive information by e-mail that an account was created for them in the reservation system. A registered employee means an employee reported by the relevant workplace as part of data collection on 8 and 9 March 2021.
Suppose the employee does not receive such an e-mail today (12 March 2021) and at the same time is subject to the obligation to prove himself/herself with a negative test upon entering the workplace and wants to pass this test at the employer. In that case, he/she must immediately contact his/her superior employee or the Personnel Office.
The reservation system is not currently intended for employees of the faculties listed in Article II. For the purpose of undergoing the test after the 90-day period, the information on the creation of the account in the reservation system will also be provided to staff who suffered from COVID-19, have the disease laboratory-confirmed and the isolation period expired, have no symptoms of COVID-19, and no more than 90 days have passed since the first positive RT-PCR test for the presence of SARS-CoV-2 or POC antigen test for the presence of SARS-CoV-2 virus antigen.
All dates for testing will be listed in the reservation system.
Employees of the faculties listed in Article II do not register for testing through the reservation system. They only follow the superior employee’s instructions.
The employee must carry an identity card, a health insurance card and an employee card, if issued, during testing.
Mass self-testing will take place mainly at faculties that are not located on the Poruba campus, i.e. at the Faculty of Economics, the Faculty of Civil Engineering and the Faculty of Safety Engineering.
Mass self-testing at faculties will begin on Monday, 15 March 2021 and further on 16 March 2021, always from 8.00 am to 1.30 pm. All employees of the particular faculty are obliged to appear for the self-testing according to their superiors' instructions and undergo it.
In this way, employees will regularly be tested at the faculties mentioned above every Monday and Tuesday from 8.00 am to 1.30 pm.
The mass self-testing will also be organised on the Poruba campus and will take place in the Sports Hall (SH on the map, not VSH near to Mensa) if the mobile testing laboratory cannot provide testing of our employees to the extent required due to capacity reasons. To be tested on the Poruba campus, the employee must register.
The employee performs the test himself/herself according to the instructions of the person responsible for supervising the self-testing.
Each employee performing the self-test is obliged to follow the authorised person's instructions or any other person involved in ensuring the correct and smooth course of testing.
If the employee has a positive antigen test, he/she will be instructed by an authorised person on how to proceed.
Testing, which will be carried out through a mobile testing laboratory, will also take place in the Sports Hall (SH on the map, not VSH near to Mensa) in the VSB-TUO complex in Ostrava-Poruba. The employee must also register for testing.
Suppose the employee has a positive antigen test. In that case, a confirmatory PCR test will be performed immediately, and he/she will be instructed by a member of the mobile laboratory or an authorised person from VSB-TUO on how to proceed.
Instructions for working with the Reservatic reservation system:
If a person enters the workplace on Monday, March 22, 2021 and passed the test on March 15, it means that in the last 7 days (15., 16., 17., 18., 19., 20 ., 21.) has undergone a test and therefore fulfills the condition set out in the emergency measure. At the same time, a person should take another test on this day if she does not meet any of the other conditions for admission. Although the emergency measure does not mention time, it is recommended that the employee be tested as soon as possible on this last day, or have been tested earlier. Of course, it is necessary to plan this adherence to the seven-day deadline in advance and order the test in time.
In connection with the epidemiological situation, on 25 February 2021 the Ministry of Health prohibits movement and residence without respiratory protective devices (nose, mouth), which is a respirator or similar device (always without an exhalation valve) fulfilling at least all technical conditions and requirements (for the product), including filtration efficiency of at least 95% according to relevant standards (eg FFP2 / KN 95), medical face mask or similar device meeting at least all technical conditions and requirements (for the product) of ČSN EN 14683 + AC in all internal university premises.
Stricter requirements for respiratory protection are set in the interiors of the VSB-TUO premises serving as a "shop" and a "service establishment", as well as in "motor vehicles for the purpose of a business trip". It is necessary to protect the airways with a properly fitted respirator meeting the requirements set by the measure (eg FFP2 or KN 95) or similar means meeting at least all technical conditions and requirements (for the product) of ČSN EN 14683 + AC, (see Article I paragraph 2 of the measure).

From 12 April 2021 until further notice, it is recommended that all managers authorized to change their employment in accordance with the Rector's Directive No. TUO_SME_03_001 to use telework for their subordinates if the employees can perform it at the place of residence, or to allow them to draw holiday. The condition is that the operating conditions of the relevant workplace allow it.
It is recommended that all managers use the online form when communicating with employees from 12 April 2021 until further notice, and only if necessary to hold meetings and other joint activities with employees in their personal presence, and to ensure that employees do not gather in premises of VSB-TUO for a purpose other than the performance of work activities.
Sessions of academic senates, scientific councils and other bodies of faculties and VSB-TUO established on the basis of Act No. 111/1998 Coll. is not limited by the Measure, or this is the exception mentioned in the Measure under Article IV. (3) and VI. (6). However, the conditions set out in the Measure must be observed during these meetings, in particular if more than 10 people participate in one place:

From March 3, 2021, the cafeterias of the University Hall and in building C will be closed, and from Monday, March 8, 2021, the canteen menu will be limited in terms of the number of meals to choose from. No persons other than employees and students will be able to eat in the canteen, so meals for pensioners and foreign diners will end. Spaces among visitors must be kept in the canteen, especially between employees from other departments. It will be possible to sit at the table for a maximum of 4 persons, any manipulation with chairs and tables is prohibited. The same regime applies to FBI and FAST dispensaries as to the canteen.
